We are seeking a compassionate, dependable, and motivated Physical Therapist (PT) to join our growing healthcare team and provide therapy services to patients throughout the Greater Los Angeles area.
This is a field-based / mobile Physical Therapist position providing evaluations and treatment in patients' homes and other appropriate community-based settings.
We welcome Physical Therapists with experience in Home Health, Outpatient Therapy, Skilled Nursing, Rehabilitation, Hospice/Palliative Care, Registry/Staffing, Geriatrics, or other community-based settings.
Our patient population may include older adults, individuals recovering from hospitalization or surgery, patients with mobility limitations, individuals at risk for falls, and patients with chronic or complex medical conditions.
For the right candidate, there may also be opportunities to work select days in an administrative/office-based role, assisting with therapy coordination, documentation review, quality assurance, and clinical operations.
Field Physical Therapist Responsibilities
- Travel to patients' homes and assigned community-based settings
- Perform comprehensive Physical Therapy evaluations and reassessments
- Assess mobility, strength, balance, coordination, endurance, range of motion, gait, transfers, and functional limitations
- Develop individualized treatment plans and functional goals
- Provide therapeutic exercises and activities
- Provide gait and balance training
- Provide transfer and mobility training
- Implement fall-prevention and home-safety interventions
- Provide strengthening, conditioning, and range-of-motion programs
- Provide neuromuscular re-education and functional mobility training as clinically appropriate
- Assess the patient's ability to safely perform activities within the home
- Recommend appropriate assistive devices and durable medical equipment when indicated
- Train patients in the proper and safe use of walkers, canes, wheelchairs, and other mobility equipment
- Provide patient and caregiver education regarding home exercise programs, fall prevention, positioning, transfers, and safety
- Monitor patient progress and modify the treatment plan as clinically indicated
- Identify changes in patient condition and communicate findings to the appropriate physician/provider and healthcare team
- Coordinate care with physicians, Nurse Practitioners, nurses, occupational therapists, speech therapists, caregivers, and other healthcare professionals
- Complete accurate and timely clinical documentation
- Follow established plans of care and applicable physician/provider orders
- Maintain patient confidentiality and comply with HIPAA, infection-control, safety, and professional standards

Qualifications
- Current and unrestricted California Physical Therapist (PT) license
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Valid driver's license and reliable transportation
- Ability and willingness to travel to assigned patient locations
- Strong clinical assessment and treatment-planning skills
- Ability to work independently in a field-based environment
- Excellent communication and patient-education skills
- Strong documentation and organizational skills
- Comfortable utilizing electronic medical record (EMR/EHR) systems
- Ability to safely perform the physical requirements associated with providing therapy in patients' homes
Preferred Qualifications
- Home Health or mobile therapy experience
- Geriatric rehabilitation experience
- Fall-risk and balance assessment experience
- Orthopedic or neurological rehabilitation experience
- Experience treating medically complex patients
- Experience with post-hospital and post-surgical rehabilitation
- Knowledge of Medicare documentation requirements

Flexible / Field-Based / Per Diem
Patient assignments are based on geographic coverage, patient needs, therapist availability, qualifications, and organizational needs.
This opportunity may be appropriate for Physical Therapists seeking per-diem, part-time, flexible, or supplemental work.
