We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will possess a strong clinical background and demonstrate exceptional patient care skills. This role involves coordinating patient care across various settings, ensuring that patients receive comprehensive services tailored to their individual needs. The Registered Nurse Case Manager will work collaboratively with healthcare providers, patients, and families to facilitate effective discharge planning and ongoing care management.
Responsibilities
- Conduct thorough patient assessments to identify needs and develop individualized care plans.
- Collaborate with interdisciplinary teams to coordinate care across inpatient, outpatient, and post-acute settings.
- Monitor patient progress and adjust care plans as necessary based on clinical evaluations.
- Facilitate discharge planning by ensuring appropriate resources are in place for continued care.
- Maintain accurate medical records in compliance with HIPAA regulations and medical terminology standards.
- Provide education to patients and families regarding treatment plans, medications, and health management strategies.
- Engage in utilization management activities to ensure effective resource allocation.
- Advocate for patients' needs within the healthcare system, addressing barriers to care as they arise.
- Stay informed about current best practices in nursing, case management, and relevant medical conditions.
Experience
Candidates should have a robust clinical background with experience in various areas including but not limited to:
- Critical care experience (ICU, PICU)
- Emergency medicine and emergency room experience
- Hospital medicine and medical-surgical nursing
- Geriatrics and behavioral health
- Infectious disease care
- Level I or Level II trauma center experience
- Outpatient clinic experience
- Post-acute care experience
- Managed care knowledge
Additional skills that are beneficial include:
- Proficiency in patient assessment techniques and vital signs monitoring
- Understanding of physiology related to acute and chronic conditions
- Familiarity with diagnostic evaluation processes
- Experience in case management principles and practices
