Job Overview
We are seeking a highly skilled and dedicated Care Case Manager III to join our multidisciplinary healthcare team in our Healthy Start program.
This role involves coordinating complex patient care plans, conducting thorough assessments, and facilitating access to community resources. The ideal candidate will possess extensive clinical experience within hospital settings, including trauma centers and acute care environments, with a focus on behavioral health and social work. This position offers an opportunity to make a meaningful impact by ensuring patients receive seamless, comprehensive care tailored to their individual needs.
Responsibilities
- Conduct detailed patient assessments to determine medical, psychological, and social needs across diverse populations.
- Develop and implement individualized care plans that incorporate medical treatment, behavioral health interventions, and social support services.
- Coordinate discharge planning and post-acute care arrangements to ensure smooth transitions from hospital to home or other facilities.
- Collaborate with healthcare providers, community organizations, and families to optimize patient outcomes and resource utilization.
- Maintain accurate documentation within electronic health records (EHR) systems while adhering to HIPAA regulations and medical terminology standards.
- Prioritize tasks effectively in fast-paced environments such as Level I Trauma Centers, ICU units, or pediatric settings.
- Provide patient education on health conditions, medication management, and lifestyle modifications to promote self-care and recovery.
Skills
- Proficiency in clinical case management, diagnostic evaluation, and utilization management within inpatient or outpatient settings.
- Knowledge of physiology, medical terminology, and behavioral health principles applicable to diverse patient populations including children and individuals with disabilities or substance use disorders.
- Experience working with electronic health records (EHR/EHR) systems; familiarity with HIPAA compliance essential.
- Strong skills in patient service delivery, client communication, task prioritization, and discharge planning coordination.
- Ability to work effectively with individuals from varied cultural backgrounds while demonstrating sensitivity and inclusivity.
