Responsibilities:
The visiting LPN is responsible for providing skilled nursing care to patients in their homes. They are responsible for the administration of all medications and treatments during the home care visits, according to the established medical and nursing care plan.
- Conducts home visits to assess patient conditions, monitor vital signs, and check for physical or mental status.
- Performs wound care and dressing changes.
- Administers and documents all medications according to established policies and procedures, physician's orders, and Department of Health regulations.
- Performs all treatments according to physician's specific order and established policies and procedures.
- Discusses the need to discontinue or change a medication of treatment with the clinical team.
- Instruct patients, caregiver and family member on disease management, medication adherence and safety measures.
Qualifications:
- Current NY State Licensed Practical Nurse
Education:
Compensation: $37.00 - $39.00 per hour
