The Patient Service Representative for the Alzheimer’s and Dementia Care (ADC) Program provides direct support to assist patients living with dementia and their caregivers. Major duties include, but are not limited to:

Acting as a liaison between patients, caregivers, physicians, and the interdisciplinary ADC Program team;

Assisting patients and caregivers in accessing services within the health system

Facilitating communication with referring physicians
- Referring patients and caregivers to appropriate health system resources

Scheduling patients for new patient and follow up appointments

Providing follow-up through telephone and EPIC/Care Connect.
Salary Range: $30.36 - $43.49/Hrly
