Duties
- Respond promptly to on-call requests for patient assessments, symptom management, and urgent care needs across diverse settings
- Provide comprehensive nursing care including vital signs monitoring, medication administration, wound care, and symptom control tailored to hospice patients
- Conduct thorough patient evaluations, including physical assessments and physiological monitoring, to inform individualized care plans
- Collaborate with interdisciplinary teams to coordinate discharge planning, manage complex cases, and ensure seamless continuity of care
- Document all patient interactions accurately in medical records while adhering to HIPAA regulations and maintaining confidentiality
- Educate patients and families on disease progression, medication use, and comfort measures to enhance quality of life
- Utilize your ICU, emergency medicine, or trauma center experience to handle acute symptoms or crises efficiently and effectively
Skills
- Proven critical care experience with a strong foundation in hospital medicine, ICU protocols, or emergency room procedures
- Experience working within Level I or Level II trauma centers or post-acute care environments such as skilled nursing facilities or outpatient clinics
- Knowledge of infectious disease management and behavioral health considerations in hospice settings
- Familiarity with medical terminology, physiology principles, and diagnostic evaluation techniques essential for accurate patient assessment
- Ability to perform comprehensive intake assessments, discharge planning, and case management for diverse patient populations including pediatrics and geriatrics
- Strong understanding of HIPAA compliance, medical records documentation, and utilization management processes
- Excellent patient service skills with a compassionate approach to end-of-life care delivery
