As a Licensed Practical Nurse MDS Specialist at Oasis at the Conch Republic, you will play a vital role in our Assisted Living Facilities, providing exceptional care and support to our residents. We are seeking a highly skilled and compassionate professional to join our team.
Responsibilities:
Conduct and document Resident Assessments, including MDS assessments, to identify individualized care needs and develop personalized care plans
Collaborate with the interdisciplinary team to develop and implement care plans that promote resident independence, dignity, and well-being
Provide direct care to residents, including administering medications, performing vital signs, and providing assistance with activities of daily living (ADLs)
Monitor and report changes in resident conditions, including signs of decline, and work with the team to implement interventions and improve outcomes
Maintain accurate and comprehensive records, including clinical notes, treatment plans, and resident data
Participate in quality improvement initiatives to improve resident care and outcomes
Requirements:
Current LPN/LVN license in the state of Florida
1+ year of experience in long-term care, assisted living, or related healthcare setting
Strong assessment and clinical skills, with ability to prioritize and manage multiple residents' needs
Excellent communication and interpersonal skills, with ability to work effectively with residents, families, and healthcare team members
Ability to work varying shifts, including days, evenings, and weekends
Compassionate and respectful attitude towards residents and colleagues
