Duties
- Conduct thorough patient assessments to determine eligibility and develop individualized hospice care plans that align with patient needs and preferences.
- Coordinate and manage patient care across inpatient, outpatient, home health, and post-acute settings, ensuring continuity and quality of care.
- Collaborate effectively with physicians, nurses, social workers, therapists, and other healthcare professionals to optimize patient outcomes.
- Oversee discharge planning, medical records management, and documentation within Electronic Health Records (EHR) systems while maintaining compliance with HIPAA regulations.
- Provide education and support to patients and families regarding hospice services, symptom management, medication administration, and advance directives.
- Monitor patient progress through vital signs assessment, diagnostic evaluation, and ongoing clinical case management to adapt care plans as needed.
- Engage in utilization management and case management activities to ensure appropriate resource utilization and adherence to health regulation policies.
Skills
- Critical care experience with a strong background in hospital medicine or emergency medicine environments.
- Extensive ICU, trauma center (Level I & Level II), or infectious disease care experience demonstrating advanced clinical skills.
- Proven expertise in managing complex cases involving geriatrics.
- Familiarity with medical terminology, physiology knowledge, and medical-surgical procedures essential for accurate patient assessment.
- Proficiency in EMR/EHR systems for documentation, medical records management, and electronic health records (EHR) management.
- Strong communication skills for client interaction, interdisciplinary collaboration, and family education.
- Knowledge of health regulation policies such as HIPAA compliance and managed care protocols to ensure legal and ethical standards are met.
- Ability to perform patient service functions including intake assessments, vital signs monitoring, discharge planning, and outpatient clinic support.
