- Assist clients with Activities of Daily Living (ADLs) dressing, grooming, and meal preparation.

Provide personal care and support in a respectful and dignified manner.

Monitor clients’ health by observing vital signs and reporting any changes or concerns to the appropriate personnel.

Implement care plans tailored to individual needs, ensuring adherence to prescribed routines.

Assist with medication administration as required.

Engage clients in social activities and community integration to promote independence and social skills.

Maintain a clean and safe living environment by performing light housekeeping tasks.

Document client progress and any incidents accurately in compliance with organizational policies.

Ability to perform heavy lifting when assisting clients; as need.

Experience working with individuals with developmental disabilities or in a group home setting is preferred.

Familiarity with Alzheimer’s care, dementia care, memory care, and hospice care practices is highly desirable.

Experience in home health or senior care environments is beneficial.
