Caring First Home Health is looking to add a PRN Speech Therapist to our Home Health team. This position will plan and provide speech therapy services to clients in their homes in accordance with the plan of care and agency policies and procedures. The Speech Therapist performs functions that require specialized knowledge, judgment and skill. This position will demonstrate the ability to communicate well with other clinicians and partners to ensure quality patient care is always delivered.
Qualifications
- Master's or Doctoral Degree in Speech-Language Pathology
- Licensed as a speech-language pathologist in Indiana
- Minimum of one (1) year experience in a health care facility or equivalent experience preferred.
- Demonstrates good written and verbal communication skills interpersonal skills.
- Licensed driver with automobile insured in accordance with state/Agency requirements. Good driving record and reliable transportation.
- Current CPR certification (company provides class, if needed)
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
- Performs speech therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.
- Completes initial assessments in forty-eight (48) hours of referral, unless other arrangements are made.
- Documents all findings, plans, interventions, and client progress (outcomes) on designated forms and submits per Agency guidelines within (48) hours.
- Communicates plans and changes to the Physician and to Case Management. As well as all other Agency staff on changes to the care plan. Completes written progress notes timely and participates in care conferences and collaboration meetings.
- Implements appropriate education based on the client’s needs and within the context of his/her environment and learning ability.
- Plans, teaches, supervises, and counsels clients and families about their speech therapy needs, in order to promote symptom management of disease process as well as overall improvement or recovery. Assist client and family with community resource needs.
- Evaluates the effectiveness of the teaching plan and makes adjustments in order to fulfill client's goals and objectives to the highest possible level.
- Uses OASIS assessment tools and outcome reports to improve therapy services and client outcomes.
- Demonstrates knowledge of safety/infection control practices by compliance with agency policies and procedures.
- Assesses safety of environment and takes initiative to help prevent accidents and promote safety.
