Job Summary
The Nurse Practitioner will deliver high-quality, patient-centered care to residents in nursing facilities, including long-term care and rehabilitation centers. This role involves managing acute and chronic conditions, coordinating care plans, and collaborating with facility staff to enhance the health and well-being of residents. The NP will serve as a key healthcare provider, ensuring residents receive timely and effective medical care in a compassionate and professional manner.
Responsibilities
- Conduct comprehensive physical exams and health assessments for residents.
- Diagnose and manage acute and chronic medical conditions commonly seen in geriatric populations.
- Monitor and evaluate changes in residents’ health status.
Care Planning and Management:
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Prescribe medications, treatments, and therapies as appropriate.
- Provide palliative and end-of-life care when needed.
Collaboration and Coordination:
- Work closely with nursing staff, physicians, therapists, and other healthcare professionals to ensure continuity of care.
- Communicate effectively with residents’ families regarding care plans, progress, and any changes in condition.
- Coordinate referrals to specialists or hospital care when necessary.
Health Promotion and Education:
- Educate residents, families, and facility staff on disease prevention, health maintenance, and treatment options.
- Promote wellness initiatives tailored to the needs of the elderly population.
Documentation and Compliance:
- Maintain accurate and up-to-date medical records in compliance with facility policies and healthcare regulations.
- Ensure adherence to state and federal guidelines, including Medicare and Medicaid requirements.
Specialized Geriatric Care:
- Address common geriatric issues such as dementia, mobility challenges, chronic pain, and polypharmacy.
- Provide wound care, infection management, and other specialized services as needed.
Qualifications
- Licensure: Active Registered Nurse (RN) license and Nurse Practitioner certification in the state of practice.
- Certifications: National certification from ANCC or AANP (e.g., AGNP, FNP).
- Experience: Prior experience in geriatric care, long-term care, or nursing facilities is highly preferred.
- Skills: Strong clinical and diagnostic skills, particularly in geriatric care.
- Excellent communication and interpersonal abilities to work with residents, families, and staff.
- Proficiency in electronic health records (EHR) systems.
Work Environment:
- Nursing homes, long-term care facilities, assisted living centers, or rehabilitation facilities.
- May involve travel between multiple facilities.
- Flexible hours, including on-call responsibilities, to meet the needs of residents.
