Job Summary
We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across various settings. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient condition through diligent observation of vital signs and overall health status
- Administer medications accurately following prescribed instructions, maintaining detailed medical documentation
- Support patients with specialized care needs such as catheter care, dementia care, Alzheimer's care, and hospice support
- Utilize Electronic Health Record (EHR) systems to document patient interactions, care plans, and medical records securely and accurately
- Implement patient transfer methods safely using equipment like Hoyer lifts or other assistive devices while adhering to home health regulations
- Provide emotional support and effective communication to clients and their families, ensuring confidentiality in compliance with HIPAA standards
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and maintaining a safe environment for the client
- Support clients with end-of-life care or long-term care needs with compassion and professionalism
- Follow all safety protocols including first aid, CPR certification requirements, and emergency response procedures
Requirements
- Proven experience working with individuals with disabilities, geriatrics, in assisted living environments, or with dementia
- Knowledge of medical terminology, medication administration, vital signs measurement, and basic life support (CPR) certification
- Familiarity with home healthcare regulations and documentation standards such as HIPAA compliance
- Ability to perform heavy lifting when necessary and operate patient transfer equipment safely
- Experience working with Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate record keeping
- Strong communication skills to effectively interact with patients, families, and healthcare teams
- Valid certification in first aid and CPR; additional certifications related to hospice or dementia care are a plus
- Ability to work independently in clients’ homes while maintaining professionalism and attention to detail
- Prior experience in home health agency settings or live-in employment preferred
- Must have a valid driver’s license and reliable transportation
