Job Description-
Patient Care:
- The contract Physician (Radiation Oncologist) shall be prepared and capable of comprehensively reviewing medical records and relevant imaging to prepare for new patient visits, follow-ups, simulations, and on treatment visits. The contractor will direct staff to obtain outside records and coordinate with non-military facilities when necessary.
- The contractor is expected to incorporate national treatment guidelines, relative medical literature, and clinical experience to formulate appropriate treatment plans. Clinical responsibility assignment wi1l dictate exact cases, but routine types of cases may consist of, but will not be limited to: breast, genitourinary, head and neck, central nervous system, gynecologic, thoracic, gastrointestinal, and hematologic malignancies as well as benign conditions such as heterotopic ossification, keloids, acoustic schwannomas, trigeminal neuralgia and graves ophthalmopathy.
Nasopharyngoscopy:
- The contractor is expected to be competent in nasopharyngoscopy.
- Stereotactic Body Radiation Therapy (SBRT) or Stereotactic Ablative Radiation Therapy (SABR).
- The contractor is expected to be competent in the use of SBRT or SABR.
Stereotactic Radiosurgery (SRS) and Fractionated Stereotactic Radiation Therapy (FSRT):
- The contractor is expected to be competent in the use of SRS and FSRT for malignant and benign intracranial disease, using both an invasive and non-invasive head ring.Total Body Irradiation {TBI}:
- The contactor is expected to be competent in patient selection, simulation, treatment planning and delivery of total body irradiation.
Other Radiation Treatment modalities:
- The contractor must be competent in variety of treatment modalities including electron treatment, three dimensional (3D) conformal treatment, intensity modulated therapy (IMRT) and volumetric modulated arc therapy (VMAT).
- The contractor must be competent in utilizing techniques including deep inspiratory breath hold (DIBH) and 4DCT.
Consent:
- The contractor will appropriately obtain informed consent prior to delivery of radiation therapy.
- They will provide advance instructions to the clinic staff on the details of the simulation including but not limited to required lab work, patient positioning, immobilization, use of contrast and scanning parameters.
- The contractor should be available to supervise the simulation, place markers as needed and delineate isocenter when appropriate or delegate to another Physician (Radiation Oncologist)
Treatment planning:
- It is the responsibility of the contractor to complete contours and review of dosimetry in a timely fashion.
- Typically contours should be completed within 3workingdaysof the simulation and the plan should be reviewed within one working day of completion.
Conferences and CME:
- The contractor shall be required to represent the Radiation Oncology Service at local Tumor Conferences as specified by the Service Chief to contribute to formulation of appropriate treatment plans, coordination of care and education of house staff.
- The contractor may attend one conference annually, up to one week in length, for the purposes of continued medical education.
Service Morning Meeting:
- The contractor will be expected to attend the Service morning meeting to review new starts, planned CT simulations, required on-treatment visits, on-treatment patient issues, treatment planning items, and to address the dosimetry, physics, radiation therapy, desk staff, and other issues.
Image guidance review:
- The contractor will review imaging daily at a minimum prior to the next fraction to provide important feedback to the radiation therapists and ensure patient safety.
- The contractor must be competent in the review and interpretation of a variety of imaging modalities including mega voltage (MV) portal films, kV/kV matching, marker match, cone beam computed tomography (CBCT}.
Weekly chart rounds:
- The contractor is expected to attend chart rounds as part of the QA process in the clinic to review all patients treated, ensure weekly visits have been completed and documented appropriately, monitor for toxicities, review imaging, and discuss patient management.
- The contractor will assess patients for enrollment in clinical trial when applicable and educate patients on clinical trial opportunities.
- The contractor will complete necessary paperwork, treatment planning and assessments for patients as stipulated by the protocols for enrollment, treatment, and follow-up.
- The contractor may not be designated as site principal investigator (PI) or assistant principal investigator but can provide clinical investigation support.
- Patient Care Documentation: The contractor shall enter reports and notes into the appropriate systems for clinical visits including history and physicals, interval histories, follow-ups, treatment summaries, and on-treatment visit notes.
- The contractor shall verify the content and correctness of all prepared and transcribed reports by validating its contents.
- These reports shall be entered within three working days of the clinical encounter.Disease Control Guidelines.
- The contractor shall adhere to the Center for Disease Control guidelines always concerning universal precautions and the Keesler Infection Control Program while providing services under this contract.
Academic Duties:
- The contractor shall participate in medical education. Prior to the performance of these duties, the COR or Clinic POC, not contractor, will confirm with DCLS attorney that each of the MTF's specific medical education requirements at that time may include non-government personnel.
- This teaching will be directed at the residents in Radiology, other Staff radiologists, residents in other departments, medical students, technologists, and other Keesler staff members.
Resident/Student preceptorship.
- The contractor shall co-sign all clinical notes produced by the residents working under his/her technical guidance.
- Prior to the performance of these duties, the COR or Clinic POC, not contractor, will confirm with DCLS attorney that each of the MTF's specific medical education requirements at that time may include non-government personnel.
Section Oversight.
- The contractor may be assigned the responsibility for medical guidance oversight of a Section of the Radiation Oncology Service which is in accordance with the radiologist's area of expertise.
- This assignment will be made by the Chief, Radiation Oncology Service in conjunction with the Chief, Department of Radiology (DOR).
- Prior to the performance of these duties, the COR or Clinic POC, not contractor, will confirm with DCLS attorney that each of the MTF's specific medical education requirements at that time may include non-government personnel.
- Be required to accomplish special quality assurance studies, no more than four per year, in accordance with schedules established by the Chief, DOR.
- The contractor shall provide advice, professional opinions and written technical studies and professional reviews upon request by the Chief, DOR in support of the various boards and committees within Keesler involved with evaluation of patient care and treatment.
- The contractor may be required to attend Service, Department or Hospital board or committee meetings in accordance with schedules established by the Chief Radiation Oncology Service (or Chief, DOR).
- Refer patients to civilian practitioners only after providing notification to, and receiving approval from, the appropriate Keesler staff in accordance with DOR and Center procedures. Keesler will retain right of first refusal.The contractor may be required to perform peer review functions and some quality assurance functions as permitted under DHA-PM 6025.13. Duties will be limited to input only and do not include decision making authority. Participation in some Quality Improvement, Risk Management, and Utilization Management Programs may be required. CME requirements shall be sufficient to satisfy requirements of appropriate licensing agencies and JC at no additional cost to the government.
Degree/Education:
- MD or DO medical degree; Successful completion of Residency in Radiation Oncology
- Certifications in addition to Basic Life Support: None, aside from those acquired during residency.
Experience: Minimum 24 months within the previous 3 years.
Board Certification: Must be Board Certified with ABR.
Licensure/Registration: Must have Mississippi License to practice medicine.
