Qualifications
- A minimum of one (1) year experience as a LPN in an acute care setting preferred.
Area of Responsibilities
- Assesses and evaluates patients on every visit.
- Implements appropriate nursing interventions to meet patient needs and change in conditions.
- Initiates plans for patient safety, involving the patient/family and community resources.
- Provides for ongoing communication with appropriate staff members to make known any changes in patient needs, schedules, physician orders, special instruction and patient updates.
- Verifies the Plan of Care prior to each visit and provides care according to physician's orders and Agency policies and procedures.
- Adheres to all Infection Control guidelines in accordance with Agency policies and procedures.
- Anticipates and responds to the needs of all customers.
- Appropriately updates/revises Medication Profiles monthly or PRN, per Agency policy procedure.
- Attends all Case/Team Conferences regarding patients and provides input for care planning, as assigned.
