As a Physical Therapist, you will provide skilled physical therapy services to patients in their homes in accordance with physician orders, Medicare guidelines, and agency policies. You’ll work independently in the field while being fully supported by our Clinical Managers, QA team, and administrative staff.
Perform comprehensive physical therapy evaluations, including SOC, Recertification, and Discharge assessments
Develop and implement individualized treatment plans based on patient needs and goals
Provide therapeutic exercises, gait training, balance training, pain management, and functional mobility interventions
Educate patients and caregivers on home exercise programs, safety, and fall prevention
Accurately complete all documentation in the EMR within required timeframes
Collaborate with physicians, nurses, therapists, and interdisciplinary team members
Participate in case conferences and care coordination as needed
Maintain compliance with Medicare, ACHC, and agency standards
Current Physical Therapist license in the State of Florida.
Minimum of one (1) year of experience as a licensed PT; home health experience strongly preferred.
Thorough understanding of Medicare documentation requirements and therapy goals in the home health setting.
Proficiency in EMR systems (experience with Kinnser/WellSky or similar is a plus).
Excellent interpersonal, communication, and time management skills.
Ability to work independently and manage a flexible per diem schedule.
Reliable transportation, valid driver’s license, and current auto insurance.
