Job Title: Assistant Program Manager / Transition Specialist

Purpose:
To use best practices and technology within a community-based residential setting to support and stabilize youth with Intellectual and Developmental Disabilities (IDD). To plan and implement Child/Family Team (CFT) interventions and schedule milieu staff to assist youth build skills needed to move to a less restrictive Intensity of Service (IOS).
Requirements:
- Bachelors level practitioner with 3-5 years of relevant (direct) experience with individuals with Intellectual and Developmental Disabilities (I/DD), or an unlicensed master's level practitioner with 1 year of the same experience.
- Valid driver's license with good driving record.
- Intermediate level of computer literacy.
- The ability to lift and transfer at least 50 pounds.
- The ability to sit, bend, squat, reach, be agile, physically pursue individuals on foot, and stand for several hours at a time.
- Must be 21 years of age or older.
- Must be able to read, write and communicate verbally (in English), and non-verbally when necessary (specific to Youth communication abilities).
- Basic knowledge of vehicle and home maintenance.

- The ability to safely and effectively execute and supervise all Personal Control Techniques.

- The ability to effectively schedule and manage milieu staff in alignment with the CSAP program design.
Duties/Responsibilities:
Works a 40-hour-per-week salaried, unlimited flexible schedule, with 25 hours per week dedicated to performing Transition Specialist duties.
In conjunction with the Program Manager:
- Shares on-call responsibility
- Supervises milieu staff and oversees the daily operational aspects of the home.
- Schedules milieu staff (according to the program model) and on-site nursing staff.
- Ensures milieu staff complete their job duties in alignment with the program design.
- Provides supervision to milieu staff during individual, group, and family interventions.
- Completes re-occurring operational tasks such as submitting evacuation drills, vehicle summaries, fire safety reports, and the environmental review.
- Reporting, monitoring, and scheduling house and vehicle maintenance.
- Ensures youth are supported in a safe, clean, and nurturing therapeutic environment.
- Provides assistance with daily living skills.
- Successfully completing required residential training, including but not limited to: New Hire Orientation; CPR and First Aid; Medication Administration; Preventing Abuse, Neglect and Exploitation; Shifting Expectations, Life Experience and Services; Life Threatening Emergencies (Danielle's Law); Stephen Komninos' Law; Crisis Management; Corporate Compliance; General Behavior; In-House Orientation; and other required trainings.
- Completes annual training, including role-specific trainings relative to Accreditation and Continuous Quality Improvement.
- Attends conferences, seminars, and other trainings relevant to the position.
- Reporting to locations other than primary assignment as needed for training, meetings, and emergencies.
- Other duties as assigned.
Transition Specialist duties:
- Completing family orientations within the first 24 hours of Youth admission.
- Reviewing and signing all required paperwork and consents within the first 24 hours of Youth admission.
- Demonstrate collaborative relationships with system partners including Care Management Organizations (CMOs) and the Division of Child Protection & Permanency (DCP&P) (if involved) and knowledge of system of care procedures and resources.
- Daily check in with program staff to obtain necessary information for transition planning.
- Contact with parent/caregiver at minimum twice per week in or (more frequently as deemed necessary) to discuss the status of their Youth's transition plan.
- Provide, as needed, on-site family psycho educational activities tied to comprehensive treatment and discharge plan monthly.
- On a weekly basis, will gather input from all team members and enter a weekly summary in the Youth's progress notes within the Children's System of Care (CSOC) electronic health record (CYBER).
