WORK SCHEDULE
Full-time or Part-time (flexible scheduling available)
KEY RESPONSIBILITIES
Perform comprehensive physical therapy evaluations
Develop and implement individualized treatment plans with measurable goals
Progress patients appropriately within established plans of care
Provide therapeutic exercises, gait training, balance training, and functional mobility interventions
Collaborate with OT, SLP, nursing staff, physicians, and the interdisciplinary team
Document evaluations, progress notes, and discharge summaries in a timely manner
Educate residents and families on mobility, safety, and continued care strategies
REQUIREMENTS / EDUCATION / EXPERIENCE
Licensed or eligible for Colorado Physical Therapist (PT) licensure
Skilled nursing or post-acute experience preferred (but not required)
Strong clinical judgment and time-management skills
A passion for patient-centered, functional rehabilitation
Team-oriented, positive, and motivated mindset
