Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly
- Administer medications accurately following prescribed schedules and medical documentation protocols
- Support patients with mobility transfers using proper patient transfer methods and equipment like Hoyer lifts or gait belts
- Maintain detailed medical records and documentation within Electronic Health Record (EHR) systems while ensuring compliance with HIPAA regulations to protect patient confidentiality
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment in accordance with home healthcare regulations
- Provide emotional support and companionship to enhance patient well-being, especially for those with dementia or experiencing end-of-life care needs
Qualifications
- Proven experience working with individuals with developmental disabilities, geriatrics, dementia care, or hospice environments is highly preferred
- Knowledge of medical terminology, catheter care, medication administration, and vital signs measurement
- Prior experience in assisted living facilities or nursing homes is advantageous; home health agency experience is a plus
- Familiarity with EMR/EHR systems for medical documentation and home health care assistance procedures
- Certifications such as First Aid, Basic Life Support (BLS), CPR are required; additional certifications in Alzheimer’s or memory care are beneficial
- Ability to perform heavy lifting safely and use patient transfer equipment effectively
- Strong communication skills to interact professionally with patients, families, and healthcare teams while maintaining patient confidentiality under HIPAA guidelines
