La Paz Hospice is seeking a dedicated and compassionate Hospice DPCS to join our team in San Diego.
Position Summary
The Hospice Director of Patient Care Services (DPCS) is responsible for planning, directing, and overseeing all clinical patient care services to ensure the delivery of high-quality, compassionate hospice care. The DPCS provides leadership to the interdisciplinary team, maintains compliance with Medicare, state, and accreditation standards, and promotes excellence in patient outcomes and staff performance.
Responsibilities:
- Direct and oversee all hospice clinical operations.
- Supervise and support nursing and interdisciplinary clinical staff.
- Ensure compliance with Medicare, state, and accreditation requirements.
- Coordinate interdisciplinary team (IDT) meetings and patient care planning.
- Monitor quality assurance and performance improvement activities.
- Oversee patient admissions, assessments, and care coordination.
- Ensure accurate, timely, and complete clinical documentation.
- Participate in staff hiring, training, coaching, and performance evaluations.
- Collaborate with the Medical Director and agency leadership to improve patient care.
- Promote a culture of compassionate, patient-centered hospice care.
Skills:
- Demonstrates ability to supervise and direct professional administrative personnel.
- Has excellent observation, verbal and written communication skills.
- Knowledge of business management, governmental regulations and Medicare CoPs.
Qualifications:
- Current, unrestricted Registered Nurse (RN) license in the State of California.
- Minimum of one (1) year of hospice management or supervisory experience preferred.
- Knowledge of Medicare Conditions of Participation, hospice regulations, and accreditation standards.
- Strong leadership, organizational, communication, and problem-solving skills.
- Valid California driver's license, reliable transportation, and current automobile insurance.
