Post Acute Specialists of Florida is seeking compassionate, tech-savvy senior care professionals to join our growing telehealth team. Guided by our mission to improve healthcare outcomes for seniors, we expand access to specialized providers through innovative telehealth solutions.
As a Telehealth Care Coordinator, you will serve as the essential in-person link between patients and medical providers during virtual care visits. By supporting patients on site and ensuring telehealth technology is properly connected and functioning, you will help create a smooth, comfortable, and positive telehealth experience for every senior we serve.
Key Responsibilities
- Enhance the Senior Telehealth Experience
- Travel to hospitals, rehabilitation centers, skilled nursing facilities, and private homes to provide in-person support for virtual post-acute care appointments
- Address patient concerns, explain the telehealth process, and ensure patient comfort and readiness during visits
Ideal Candidate Profile
- CNA or MA with senior care experience
- Patient, empathetic, and confident when supporting seniors with diverse needs
- Comfortable learning and adapting to new technologies and systems
- Experienced in post-acute care environments, such as SNFs, rehab centers, or hospitals
- Knowledgeable in basic healthcare processes, terminology, and HIPAA standards
- A strong communicator with excellent problem-solving skills
Required Qualifications
- Current CNA or MA certification with at least 2 years of recent experience
- PCT or Home Health experience is a plus
- BLS Certification
- Ability to accurately measure and record vital signs
- TB test within the past 6 months
- High School Diploma or GED
- Clear criminal background check
- Valid driver’s license, reliable insured vehicle, and clean driving record
