Coordinates and participates in activities related to Care Management services to family members and caregivers. HARP (Health and Recovery Program): The HARP team is a portion of the Care Management Health Home spectrum that specializes and supports patients who meet the NYS behavioral health high-risk criteria guidelines and provides them with higher intensity case management.
