Your Responsibilities:
- Supervise and manage a Home Health nursing team, ensuring high-quality patient care delivery aligned with organizational policies and healthcare standards.
- Develop, implement, and evaluate individualized care plans for patients across diverse populations.Coordinate patient assessments, medical documentation, and clinical evaluations utilizing the HCHB EMR.
- Oversee compliance with HIPAA regulations, state healthcare laws and Medicare/Medicaid guidelines.
- Facilitate staff training on clinical procedures such as IV infusion, catheter care, ventilator management, sterile technique, and basic life support (BLS).
- Lead discharge planning efforts and collaborate with interdisciplinary teams to ensure seamless transitions of care.
Your qualifications:
- Current CA Registered Nurse (RN) license.
- At least one year of proven management or nurse leadership experience supervising clinical teams in skilled or Medicare-certified Home Health or related settings.
- Working knowledge of any EMR/EHR systems such as HCHB, Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with ICD/10 documentation standards.
- Working knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques.
- Ability to perform comprehensive patient assessments including vital signs, physical examinations, specimen collection/processing, and medication administration.
- Knowledge of HIPAA regulations, and skilled Home Health compliance standards.Excellent communication skills for managerial coordination and team management; adept at managing field clinican teams in diverse environments.BSN degree and a few years working in a Home Health company is preferred.
