Job Overview
We are seeking a highly skilled and strategic Director of Health Home to lead our integrated care programs focused on comprehensive patient-centered services. The ideal candidate will possess extensive experience in healthcare management, with a strong background in hospital operations, behavioral health, and case management. As the Director, you will oversee the development, implementation, and evaluation of health home initiatives designed to improve patient outcomes, enhance care coordination, and ensure compliance with regulatory standards. This leadership role requires a proactive approach to managing multidisciplinary teams and fostering innovative solutions within complex healthcare environments.
Responsibilities
- Lead the design and execution of health home programs that integrate physical health, behavioral health, and social services to support patient needs effectively.
- Oversee clinical operations across hospital settings including Level I Trauma Centers, Level II trauma centers, PICUs (Pediatric Intensive Care Units), and emergency departments to ensure high-quality patient care delivery.
- Supervise multidisciplinary teams comprising nurses, social workers, addiction counselors, and healthcare providers to promote collaborative care planning and case management.
- Ensure compliance with HIPAA regulations, NCQA standards, Medicare requirements, and other relevant healthcare policies through rigorous documentation review and data collection processes.
- Develop strategies for discharge planning, utilization review, and medical records management to optimize patient transitions across care settings.
- Foster a culture of continuous improvement by analyzing clinical data, monitoring quality metrics, and implementing evidence-based practices aligned with behavioral health and managed care standards.
- Provide leadership in staff supervision, training on medical terminology, physiology, anatomy knowledge, and behavioral health counseling techniques to enhance team expertise.
Experience
- Extensive hospital experience including acute care environments such as ICU (Intensive Care Unit), emergency medicine departments, and pediatric units like PICU.
- Proven management experience supervising multidisciplinary healthcare teams within hospital or home health settings.
- Familiarity with Level I Trauma Centers and Level II trauma center operations is highly desirable.
- Strong background in nursing or healthcare management with knowledge of discharge planning, medical records documentation review, and utilization review processes.
- Experience working within managed care organizations or behavioral health programs is preferred.
- Knowledge of social work practices related to case management and behavioral therapy is advantageous.
- Demonstrated leadership skills with the ability to coordinate complex projects while maintaining compliance with HIPAA regulations and NCQA standards. Pay: $100,000.00 - $103,000.00 per year
