Prior experience in MDS is required
Experience in MDS/PDPM
Able to complete assessments and care plans
Ensure accurate and timely completions
OBRA calendar
Monitoring that all departments complete their assessments timely
Monitoring and auditing clinical records
Being a team player
Adheres to federal, state, and local regulations
Keeps appraised of changes and updates in nursing practice
Functions as a member of the nursing team
Develop and maintain a good working rapport with interdepartmental personnel, as well as other departments within the facility to ensure that services and activities can be adequately maintained to meet the needs of the residents
Routinely assess residents according to pre-set schedules, or on an “as needed” basis for condition changes, hospital return, etc.
Responsible for accurate observation, assessment and communication of condition changes to appropriate personnel
Oversee completion, accuracy and updating of behavioral, mood, rehabilitation, discharge, activities, and dietary care plans
Assures completion of assessments and Summaries by other departments such as Social Work, Activities, Dietary and obtain corresponding signatures
