Northern Neck Senior Care Community and Three Rivers Health & Rehab Center are seeking a compassionate and dedicated Occupational Therapist (OT) to join our in-house rehabilitation team. Whether you're an experienced clinician or a new graduate, we welcome motivated professionals who are passionate about helping patients regain independence and improve their quality of life.
Key Responsibilities
- Evaluate residents and develop individualized occupational therapy treatment plans.
- Provide skilled occupational therapy services to individuals with temporary or long-term physical and functional limitations.
- Help patients improve their ability to perform activities of daily living (ADLs) and maximize independence.
- Modify treatment plans based on patient progress and clinical needs.
- Educate residents, families, and caregivers on therapeutic techniques and home exercise programs.
- Collaborate with physicians, nursing staff, and the interdisciplinary care team to ensure high-quality, patient-centered care.
- Complete evaluations, progress notes, recertifications, discharge summaries, and all required documentation in accordance with company policies and state and federal regulations.
- Maintain compliance with HIPAA and all applicable regulatory standards.
Qualifications
- Graduate of an accredited Occupational Therapy program.
- Current Virginia Occupational Therapist (OT) license in good standing.
- Experience in skilled nursing, long-term care, or geriatric rehabilitation is preferred but not required.
- New graduates are encouraged to apply.
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
