We are seeking an experienced Registered Nurse Clinical Manager who is ready to advance into a Director of Clinical Services (DCS) leadership role.
While the position carries the title of Director of Clinical Services, our ideal candidate has a strong background as a Clinical Manager within a Medicare-certified home health agency, with extensive experience overseeing daily clinical operations, managing clinicians, coordinating patient care, ensuring regulatory compliance, and driving quality outcomes.
This is a hands-on leadership position responsible for supporting and guiding the clinical team while ensuring exceptional patient care and operational excellence.
Primary ResponsibilitiesClinical Management & Patient Care Oversight
- Oversee the daily clinical operations of a busy Medicare-certified home health agency.
- Provide direct leadership and support to Registered Nurses, Licensed Practical Nurses, Therapists, Home Health Aides, and other clinical staff.
- Supervise patient care from admission through discharge, ensuring quality, continuity, and timely service delivery.
- Review referrals, admissions, recertifications, resumptions of care, transfers, and discharges to ensure compliance and clinical appropriateness.
- Ensure all patient care is delivered according to physician orders, agency policies, and Medicare Conditions of Participation.
- Monitor clinician caseloads, productivity, scheduling, and visit utilization to maximize efficiency and patient outcomes.
- Collaborate with Intake, Scheduling, and Operations to coordinate timely admissions and ongoing patient care.
- Serve as a clinical resource for field clinicians by providing guidance, problem-solving, and support for complex patient situations.
- Promote interdisciplinary collaboration to ensure coordinated, patient-centered care.
Case Management & Care Coordination
- Oversee case management activities to ensure patients receive appropriate services and achieve optimal clinical outcomes.
- Monitor patient progress and intervene when changes in condition or care plans require additional clinical oversight.
- Communicate regularly with physicians regarding orders, plan of care changes, medication concerns, and patient status.
- Collaborate with referral sources, hospitals, physician offices, and community partners to facilitate smooth transitions of care.
- Ensure timely follow-up on physician orders, verbal orders, and outstanding documentation.
OASIS & Clinical Documentation
- Review and audit OASIS assessments for accuracy, completeness, and regulatory compliance.
- Ensure timely completion of Start of Care, Recertification, Resumption of Care, Transfer, and Discharge documentation.
- Monitor documentation quality to support reimbursement, survey readiness, and quality outcomes.
- Coach and mentor clinicians on documentation standards, Medicare requirements, and best clinical practices.
- Maintain compliance with documentation timelines and agency performance expectations.
Quality Assurance & Regulatory Compliance
- Lead Quality Assurance and Performance Improvement (QAPI) initiatives.
- Conduct routine chart audits and clinical record reviews to ensure compliance with Medicare, AHCA, and accreditation standards.
- Monitor quality indicators, patient outcomes, hospitalization rates, infection prevention, and performance metrics.
- Identify opportunities for process improvement and implement corrective action plans.
- Prepare the agency for state surveys, accreditation reviews, and regulatory audits.
- Ensure ongoing compliance with Medicare Conditions of Participation, Florida regulations, agency policies, and professional standards.
Staff Leadership & Development
- Supervise, mentor, coach, and support Clinical Managers, Registered Nurses, Licensed Practical Nurses, Therapists, and other clinical personnel.
- Assist with hiring, onboarding, orientation, training, competency evaluations, and performance management.
- Conduct staff meetings and clinical education programs.
- Foster accountability, teamwork, and a culture focused on clinical excellence.
- Support employee engagement, retention, and professional development.
Operational Leadership
- Collaborate with executive leadership to improve clinical operations and agency performance.
- Monitor staffing needs, scheduling efficiency, productivity, and resource utilization.
- Participate in strategic planning and operational initiatives that support agency growth.
- Assist in developing policies, procedures, and clinical workflows that improve efficiency and patient outcomes.
- Serve as a liaison between clinicians, physicians, referral sources, patients, caregivers, and agency leadership.
QualificationsRequired
- Current Florida Registered Nurse (RN) license in good standing.
- Minimum one (1) year of supervisory or management experience in a Medicare-certified home health agency.
- Previous Clinical Manager experience is required.
- Strong experience supervising clinicians, coordinating patient care, and managing daily clinical operations.
- Demonstrated experience with clinician scheduling, caseload management, and workforce coordination.
- Extensive knowledge of Medicare Conditions of Participation and home health regulations.
- Experience reviewing OASIS assessments and clinical documentation.
- Strong understanding of Quality Assurance and Performance Improvement (QAPI).
- Experience communicating with physicians regarding patient care, orders, and care coordination.
- Strong leadership, organizational, communication, and critical-thinking skills.
- Experience using WellSky/Kinnser or similar home health EMR systems.
Preferred
- Experience serving as a Director of Clinical Services or Assistant Director of Clinical Services.
- Experience with ACHC, CHAP, or Joint Commission accreditation surveys.
- Multi-site home health leadership experience.
- Experience improving clinical outcomes, survey readiness, and regulatory compliance.
