Oasis Pavilion Nursing and Rehabilitation Center is seeking an SLP for our Skilled Nursing Facility. Oasis Pavilion is a 134-bed skilled nursing facility located in Casa Grande, AZ.

Essential functions
1. Assess, diagnose, and create personalized treatment plans for patients with speech, language, cognitive, and swallowing disorders.
- Use cutting-edge techniques and evidence-based practices to help patients achieve their fullest potential.
2. Plans, direct, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and/or swallowing or cognitive problems.
- Administers, scores, and interprets specialized speech tests.
- Instructs residents/patients, families, and caregivers to monitor speech and provide ways to practice new skills.
3. Completes required forms and clinical records in accordance with company policy and state and/or federal regulations.
- Documents in-patient records in an accurate and timely manner.
- Devises special tools and treatments for the specific needs of patients.
4. Collaborate with a multidisciplinary team, including physical therapists, occupational therapists, and nursing staff to deliver holistic care.
- Educate patients, families, and caregivers on treatment strategies and progress, fostering a supportive environment for recovery.
5. Document treatment plans and patient progress with precision and accuracy.
6. Provides comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting.
- Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment.
- Maintains resident/patient privacy pursuant to HIPAA.
- Ensures punctuality and regular attendance for assigned shifts.
Qualifications
Education:
Bachelor of Science degree in Speech Language Therapy, required.
Licenses/Certification:
Current license in Speech Language Therapy, as required by State law.
Experience:
Six months experience in a long-term care environment or equivalent, preferred.
Salary: $135K to $140K
