This position is paid on a per-visit basis. The compensation reflected on this posting, is an estimate of hourly compensation.
Responsibilities
- Make home visits to clients in the community, as well as Senior Living, Assisted Living, and Independent Living facilities.
- Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
- Evaluate the home environment for hazards or barriers to more independent living.
- Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
- Continually assess and revise the physical therapy care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
- Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
- Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
- Prepare and submit clinical and progress summaries based on identified and attained goals
- Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
- Review and sign the physical therapy assistant’s notes and ensure to communicate with the physical therapy assistant any changes in the care plan/treatment plan.
Qualifications
- Doctor of Physical Therapy degree in an accredited physical therapy curriculum
- Certification in Physical Therapy and valid state license to practice
- Active drivers license, registration, and insurance
- CPR

Pay: $95.00 - $105.00 per hour
