SUMMARY
As a Nurse Practitioner in Geriatrics, you’ll provide comprehensive primary and acute care to older adults across both Long-Term Care and Assisted Living settings. This role offers the opportunity to build meaningful relationships with patients while managing a diverse range of acute and chronic conditions. You’ll work alongside a supportive team with strong onboarding and collaboration with nursing, respiratory therapy, facility staff, and experienced geriatric physicians.

ATTRIBUTES
Prior experience as a provider is preferred
Open to new graduate NPs with a strong nursing background, particularly in med-surg or primary care

RESPONSIBILITIES
Patient Care. Provide primary and acute care for geriatric patients, including assessment and management of acute and chronic conditions, physical examinations, diagnosis, treatment planning, medication management, and follow-up care
Long-Term Care. Provide care 2–3 days per week at Unity Living Center, including management of long-term care residents and patients requiring long-term ventilator support in collaboration with respiratory therapy
Assisted Living. Provide primary care 2–3 days per week to residents in assisted living settings, working closely with facility staff and the Geriatric Medical Director
Care Coordination. Collaborate with nursing, respiratory therapy, facility staff, physicians, and other members of the care team to coordinate comprehensive patient care
Documentation. Maintain accurate medical records, complete required documentation, and monitor patient response to treatment

LICENSES / CERTIFICATIONS:
NP - Nurse Practitioner Certification - New York State Education Department (NYSED), RN - Registered Nurse - New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE:
$100,000.00 - $150,000.00
