Job Summary
Cares In Motion is seeking compassionate and dedicated Supportive Home Care Assistants to provide exceptional care and support to individuals in their homes and community settings in Burnett, Polk and St. Croix Counties. This role involves assisting clients with daily activities, ensuring their safety, and promoting independence. Ideal candidates will have a strong background in senior care, disability support, or home health services, with a focus on personalized patient care, but is not required. Training program available! The position offers an opportunity to make a meaningful difference in the lives of those requiring assistance with daily living and health management.

Webster/Grantsburg area:
7 Days a Week
Days/Evenings
Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Provide medication reminders following care plans
- Observe client conditions to report any changes promptly
- Help prepare nutritious meals tailored to individual dietary needs and assist with meal planning
- Use specialized equipment such as Hoyer lifts or client lifts to safely transfer clients
- Assist client's with transportation to medical appointments, grocery shopping or fun outings.
- Maintain accurate documentation of care provided, adhering to HIPAA regulations and confidentiality standards
- Support clients with cognitive impairments such as dementia or Alzheimer's disease through memory care techniques
- Assist with light housekeeping tasks including cleaning, laundry, and maintaining a safe environment
- Engage clients in social activities and provide companionship to promote emotional well-being
- Follow prescribed care plans developed by our interdisciplinary team
- Ensure compliance with all safety protocols, including CPR and first aid procedures
Experience
Well suited candidates enjoy working 1:1 with people in their private homes and have a strong desire to improve the client's day through socialization, maintaining a safe environment and increasing the client's independence at home. Well suited candidates should possess relevant experience in home health, assisted living, nursing homes, or group home settings. Prior experience working with individuals with developmental disabilities, autism, dementia, or hospice care is highly valued but not necessary, we will provide training to the right candidate.
