SAJE Strategic Solutions is seeking a precise, detail-oriented, and highly skilled RN MDS Coordinator to manage and oversee the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) process within a 100+ bed skilled nursing facility.
In this role, you will utilize your RN clinical expertise to lead the interdisciplinary team (IDT), ensuring accurate clinical assessments, strict regulatory compliance, and optimal reimbursement under the Patient Driven Payment Model (PDPM). The ideal candidate brings a strong professional nursing background, deep knowledge of CMS guidelines, and a data-driven approach to case-mix management.
Key Responsibilities1. Assessment & Care Planning
- Coordinate and manage the timely completion and electronic submission of all OBRA and PPS assessments in accordance with state and federal schedules.
- Leverage RN-level clinical judgment to review complex medical charts and collaborate directly with physicians, nursing leadership, therapy, and social services to ensure holistic, accurate resident profiles.
- Direct the development, implementation, and evaluation of individualized, resident-driven care plans.
- Lead weekly interdisciplinary Care Plan and utilization review (UR) meetings.
2. Reimbursement & Compliance
- Analyze clinical documentation to support accurate ICD-10 coding and proper PDPM classification.
- Monitor Quality Measures (QMs) and Five-Star rating metrics, identifying critical areas for clinical documentation improvement (CDI).
- Maintain full audit readiness and assist facility leadership during state surveys and Medicare/Medicaid audits.
- Actively participate in the facility's Quality Assurance Performance Improvement (QAPI) initiatives.
3. Team Collaboration & Education
- Act as the primary resident assessment expert and clinical resource for the facility staff.
- Provide ongoing training and clinical education to floor nurses regarding documentation requirements, ADL tracking, and functional scores.
Qualifications & Requirements
- Licensure: Active, unrestricted California Registered Nurse (RN) license.
- Experience: Minimum 2 years of direct MDS experience within a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting.
- Technical Expertise: Comprehensive understanding of MDS 3.0, RAI User Manual guidelines, PDPM, and CMS compliance standards.
- Software: Proficiency with long-term care Electronic Medical Records (EMR) software (e.g., PointClickCare or similar platforms).
- Certifications: RAC-CT (Resident Assessment Coordinator-Certified) through AAPACN is highly preferred.
- Skills: Advanced clinical assessment acumen, exceptional attention to detail, and top-tier communication skills to bridge clinical data and financial compliance.
