Duties
- Assist patients with activities of daily living (ADLs) such as bathing, grooming, dressing, and mobility to promote independence and comfort
- Monitor patient condition through regular observation of vital signs and overall well-being, reporting changes promptly to nursing staff
- Provide emotional support and companionship to patients, respecting their personal preferences and cultural backgrounds
- Maintain accurate medical documentation within electronic health record (EHR) systems, ensuring confidentiality in compliance with HIPAA standards
- Support end-of-life care by creating a calm environment, assisting with pain management, and respecting patient dignity at all times
- Assist with transfers using patient transfer methods such as Hoyer lifts or other equipment while ensuring safety protocols are followed
Experience
- Prior experience working with geriatric patients in hospice, nursing homes, assisted living facilities, or home healthcare settings is highly preferred
- Demonstrated knowledge of medical terminology, catheter care, and vital signs monitoring
- Experience working with individuals with developmental disabilities or dementia care is advantageous
- Familiarity with EMR systems and medical documentation practices required for accurate record keeping
- Certification in CPR and basic life support (BLS), along with first aid training, is essential for emergency preparedness
- Proven ability to handle heavy lifting safely and perform patient transfers using appropriate techniques and equipment
- Previous roles involving home health care assistance or live-in employment experience are a plus
