Job Overview
We are seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments to determine care needs, including vital signs, physiological status, and symptom management.
- Develop individualized care plans in collaboration with physicians, social workers, and other healthcare professionals to meet each patient’s unique needs.
- Manage and coordinate hospice services, including medication administration, symptom control, and discharge planning.
- Monitor patient progress regularly through clinical evaluations and adjust care plans accordingly to optimize comfort and safety.
- Educate patients and families on disease progression, medication use, and available support resources to empower informed decision-making.
- Maintain accurate medical records and documentation within electronic health records (EHR) systems while ensuring compliance with HIPAA regulations.
- Facilitate communication among multidisciplinary teams to ensure seamless delivery of hospice care services across inpatient, outpatient, home health, and post-acute settings.
Skills
- Proven experience in critical care settings such as ICU, emergency medicine, or trauma centers with a strong understanding of physiology and medical terminology.
- Knowledge of hospice care principles, post-acute care processes, discharge planning, and home health services.
- Skilled in patient assessment, clinical case management, utilization management, and diagnostic evaluation.
- Familiarity with electronic health records (EHR/EHR) systems and medical records management practices.
- Excellent client communication skills with the ability to collaborate effectively with healthcare professionals across hospital medicine, behavioral health, geriatrics, pediatrics, and managed care environments.
- Strong understanding of health regulation policies including HIPAA compliance and medical-surgical standards.
- Ability to manage multiple priorities efficiently within inpatient or outpatient settings while maintaining high-quality patient service standards.
