Responsibilities
- Conduct comprehensive patient assessments including vital signs, physical examinations, and health histories to inform individualized care plans.
- Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA and infection control standards.
- Utilize EMR (Electronic Medical Records) systems such as Cerner or Athenahealth to document patient information thoroughly and efficiently.
- Support complex procedures such as IV infusions, catheterizations, suturing, and tube feeding with precision and aseptic technique.
- Collaborate with multidisciplinary teams to coordinate discharge planning, case management, and follow-up care for optimal patient outcomes.
- Manage acute pain and provide palliative care in hospice or memory care settings with sensitivity and professionalism.
- Assist in diagnostic evaluations including laboratory specimen collection, processing, and interpretation of results like ICD coding or ICD-10 documentation.
Skills
- Proven experience working with diverse populations including pediatrics, geriatrics, individuals with developmental disabilities, or those requiring specialized memory or dementia care.
- Familiarity with EMR/EHR systems such as Cerner or Athenahealth; proficiency in medical documentation and coding (ICD-9/ICD-10).
- Strong clinical skills including IV insertion, phlebotomy, sterile processing, airway management, and ventilator support.
- Knowledge of critical care environments such as ICU or Level I Trauma Centers; experience in emergency medicine or trauma medicine is highly valued.
- Ability to perform complex procedures like cardiac catheterization preparation, sonography, gastrostomy management, or ostomy care confidently.
- Excellent patient assessment skills combined with physiology knowledge to support acute care needs across hospital medicine or outpatient settings.
- Demonstrated capability in infection control practices, triage procedures, and working within NCQA standards for quality healthcare delivery.
- Effective communication skills for working with children, seniors in assisted living or nursing home environments, and individuals with disabilities requiring tailored health coaching or behavioral health support.
