Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to support their independence and comfort
- Monitor patient health by observing vital signs and reporting changes promptly to healthcare providers
- Support medication management by administering prescribed medications following medical instructions and maintaining accurate medical records
- Aid with mobility and transfers using patient transfer methods and equipment like Hoyer lifts to ensure safety for both patient and caregiver
- Provide companionship and emotional support to reduce feelings of isolation, especially for geriatric patients or those with dementia or Alzheimer's disease
- Maintain a clean and safe environment by assisting with light housekeeping tasks including laundry, cleaning, and meal preparation in accordance with home health regulations
- Document all patient interactions accurately using Electronic Health Record (EHR) systems while maintaining strict confidentiality in compliance with HIPAA standards
Requirements
- Proven experience working with individuals with developmental disabilities, dementia care, or in assisted living or nursing home settings is highly preferred
- Certification in First Aid and Basic Life Support (BLS), including CPR training additional certifications such as hospice care or end-of-life care are a plus but not needed
- Familiarity with home health care regulations, HIPAA compliance, and medical documentation practices necessary for quality patient care
- Ability to lift heavy objects safely and perform patient transfers using appropriate techniques and equipment like Hoyer lifts or patient transfer methods
- Strong communication skills for effective client interaction and collaboration with healthcare teams
- Valid driver’s license preferred for travel between client locations; experience working with home health agencies or live-in employment is advantageous
