Role Overview: We are seeking a qualified and compassionate Home Health Licensed Practical Nurse (LPN) to join our team. The ideal candidate will provide personalized nursing care to patients within their homes, ensuring their comfort and well-being while assisting them in their daily activities. The LPN or LVN will play a crucial role in collaborating with healthcare teams to implement and manage care plans tailored to individual patient needs. Every other weekend shift requirements.

Key Responsibilities and Duties:
- Patient Care: Conduct comprehensive assessments of patients' health conditions, needs, and home environment to establish effective nursing care plans.
- Patient Education: Educate patients and their families on care plans, medication management, and health maintenance strategies to promote optimal health and wellbeing.
- Health Promotion: Encourage healthy lifestyle choices and disease prevention, including nutrition, exercise, and social engagement initiatives.
- Administer Treatments: Provide clinical nursing care, including administering medications, wound care, and other medical treatments as prescribed.
- Monitoring and Documentation: Monitor patients' vital signs and conditions, documenting changes accurately in medical records and ensuring up-to-date information is communicated to the healthcare team.
- Compliance: Maintain knowledge of current healthcare regulations and policies to ensure compliance with state and federal guidelines as well as company policies.
- Emergency Response: Respond proactively to patient emergencies and changes in condition, providing supportive interventions and contacting appropriate medical personnel when necessary.
- Professional Development: Stay current with developments in nursing practices, medical treatments, and home health technologies through ongoing education and training.
- Interdisciplinary Collaboration: Collaborate with physicians, physical therapists, occupational therapists, social workers, and other members of the healthcare team to coordinate comprehensive care.
Compliance and Regulatory Requirements:
- Adhere to all Federal and State home health laws, regulations, survey readiness preparation, and compliance per Agency policy
- Comply with all clinical department and agency policy and procedures
Education and Experience Qualifications:
- Current and valid Licensure as a Practical or Vocational Nurse in TN (LPN).
- Previous experience in home health care, geriatrics, or a similar field preferred.
- Basic Life Support (BLS) certification (required).
- Familiarity with Electronic Medical Records (EMR) and documentation processes.
- Strong interpersonal and communication skills to effectively collaborate with patients, families, and healthcare teams.
- Empathy, patience, and a passion for providing high-quality patient care.
- Ability to work independently and manage time effectively in a home environment.
- Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare team members.
- Ability to work independently and manage time effectively in a home health setting.
- Proficiency in using electronic health records (EHRs) and medical documentation systems.
- Valid driver’s license and access to a reliable vehicle.
- Background check and drug screening required.
Compensation
- $28.00 - $32.00/hour for LPN
