WHO WE ARE: Quality Community Health Care (QCHC) is a Federally Qualified Health Center (FQHC) operated health care center since 1980. We provide service at 3 locations in Philadelphia, PA, managing the health and wellness of patients from infants through senior citizens. We practice medicine as a partnership between us and our patients.
POSITION PURPOSE: To provide comprehensive acute and chronic care to all adult and pediatric patients.
ESSENTIAL FUNCTIONS AND BASIC DUTIES:

Provide primary care services to Pediatric and Adult patients.

Assist in triaging all patients presented to the clinic for services.

Ensure all protocols for patient care and clinic operations are followed.

Work closely with the adult and pediatric physicians to ensure all services are available to all patients.

Under the supervision of the Adult and Pediatric Physician, perform health evaluations for all patients who are presented to the clinic.

Provide emergency assistance when necessary.

Maintain logs of all patients who are screened or medically evaluated.

Counsel parents and teachers, when necessary, on aspects of the child’s physical and mental health.

Ensure that all patient records are maintained in the prescribed manner.

Ensure patient confidentiality at all times.

Adherence to Occupational Safety and Health Administration (OSHA) and Clinical Laboratory Improvement Amendments (CLIA) regulations.

Participate in administering immunizations, protocols, and procedures as indicated by the American College of Physicians (ACP) and Advisory Committee on Immunization Practices (ACIP).

Participate in weekly conferences and peer reviews.

Adhere to QCHC policies and procedures.

Participate in developing, implementing, and ensuring adherence to QCHC’s Bioterrorism Plan, according to guidelines set forth by the Pennsylvania Department of Health (PADOH) and the Center for Disease Control (CDC).

Participates in the Provider's Wheel of Champions Program as assigned.

Travel to work at other QCHC medical sites as needed.

Administer immunizations and injections.

Providing referrals internally and externally.

Ordering medications that a physician authorizes.

Performing routine diagnostic examinations.

Responsible for documenting in the Electronic Medical Record (EMR) promptly.

Participate in the after-hour on-call rotation roster.

MPA-LIC - PA Medical Physician Assistant License (Required)

NCCPA - Certified Physician Assistant (Required)
Experience:

Two (2) years of experience as a physician assistant (Required).
