Job Overview
We are seeking a dynamic and compassionate Case Manager/Group Facilitator to join our dedicated healthcare team. Responsibilities
- Develop individualized care plans that incorporate discharge planning, post-acute care coordination, and utilization management strategies
- Facilitate engaging group sessions for patients focused on behavioral health, addiction counseling, mental health support, or education about medical conditions
- Collaborate with interdisciplinary teams including social workers, nurses, physicians, and therapists to optimize patient outcomes
- Manage electronic health records (EHR) and medical records with accuracy while adhering to HIPAA regulations and confidentiality standards
- Assist with intake procedures for new patients, ensuring proper documentation and understanding of medical terminology and diagnostic evaluations
Requirements
- Familiarity with discharge planning, utilization management, diagnostic evaluation, and medical records management
- Knowledge of medical terminology, physiology, and electronic health record (EHR) systems such as EMR/EHR platforms
- Certification in Basic Life Support (BLS) and CPR is recommended; additional certifications in addiction counseling or behavioral health are a plus
- Excellent task prioritization skills coupled with outstanding patient service and communication abilities
- Ability to work effectively within managed care frameworks while maintaining compliance with HIPAA regulations

Pay: $45,000.00 - $50,000.00 per year
