POSITION SUMMARY
The Lead Case Manager provides case management at the Commons Permanent Supportive Housing (PSH) community for Veterans, families, and individuals. The Lead Case Manager acts as a lead to provide support to the team of case managers and housing assistants, and will also carry a caseload of clients. This position reports to the Director of Supportive Housing.
DUTIES AND RESPONSIBIITIES:
Program Coordination
- Responsible for supporting the Director of Supportive Housing with the oversight The Commons, a Permanent Supportive Housing (PSH) community for Veterans, families, and individuals.
- Effectively support other staff to include supporting core values, addressing zero tolerance behaviors, and maintaining active communication with employees.
- Work in conjunction with the property management company to ensure building operations are followed in accordance with policies and procedures.
- Review and assist with guidance various proposals and contracts.
- Assist in coordinating, managing, and monitoring group work, ensuring that life skills groups are provided, documented, and billed regularly.
- Assist supervisor with training new and incumbent staff, evaluate training needs, implement policies and procedures, and provide feedback to the supervisor.
- Other duties as assigned.
Case Management
- Serve as Case Manager for client/residents while carrying a caseload: work with each client to set goals for healthy individual growth with an emphasis on maintaining housing; monitor progress in attaining goals; monitor adherence to all Homeward Pikes Peak policies and procedures specific to The Commons.
- Provide transportation for clients to appointments when necessary.
- Coordinate with other community service agencies to access services for clients as needed.
- Maintain accurate records, which include client charts as well as internal and external reports.
- Develop Individualized Services Plans for clients.
- Assist in maintaining systems to ensure client records and databases are up to date, ensure that client interactions are timely and appropriately documented in the HMIS system, and adhere to appropriate confidentiality standards relative to data. Ensure Reliatrax is utilized regularly to support the billing of supportive services.
- Assist with maintaining updated policies and procedures, training manuals for assigned programs, and assist with overseeing the creation and implementation of yearly training for all staff.
- May attend appointments with clients.
- Support clients’ vocational choices and assist them in choosing a job that matches their strengths, overcoming job-related anxiety by reviewing job applications, and providing interview tips.
- Assist clients in building social skills in the community that will enhance job acquisition and tenure.
- Identify any problems, challenges, or obstacles in coordination of services, identify potential solutions, and resolve such problems, challenges, or obstacles in a timely manner.
- Document intervention activities in an accurate and timely manner and seek consultation from supervisor or other staff as necessary.
- Other duties as assigned.
HPP TEAM RESPONSIBILITIES
- Attend and participate in staff meetings and monthly client luncheons.
- Attend / participate / interact with donors, attend community events, or fundraising as requested.
- Participate in weekly supervision.
- Integrate with the overall organization and collaborate with the community.
- Produce documentation timely as requested for audits and audit resolutions.
- Maintain office environment to professional standards and display abundant teaming skills.

KNOWLEDGE, SKILLS, ABILITIES:

- Proficiency in general computer skills and Microsoft Office applications including Word, Excel, and Outlook; word processing, spreadsheets, CRM databases, and other case management platforms.
- Understanding of HIPAA regulations and standards regarding privacy and confidentiality in the provision of therapy services and medical billing.
- Demonstrated knowledge and training in generalized health care standards, substance abuse recovery process and treatment, mental health care, and trauma informed care skills.
- Knowledge of basic care coordination, medical/hospital and/or community resources.
- Ability to work comfortably among clients with or without substance abuse disorder, who are referred from city and county parole/probation departments, department of social services, and other relevant agencies.
- Ability to use critical thinking to prioritize, make analytical decisions, and problem solve.
- An understanding of issue related to homelessness, mental illness and substance abuse.
- Detail oriented and organized, and ability to maintain confidentiality.
- Ability to maintain professional boundaries with clients.
- Self-starter and ability to work independently with minimal supervision.
- Ability to occasionally lift or move up to 25 lbs.
MINIMUM QUALIFICATIONS:
- Bachelor’s level degree in Social Services required.
- 3 – 5 years of experience in the human service field providing direct client supportive services.
- First Aid / CPR certified, or willing to obtain within 3 months of hire.
- Experience working directly with people from diverse socioeconomic and ethnic backgrounds, ability to communicate effectively, crisis intervention, and harm reduction.
- Understanding of regulations and standards regarding privacy and confidentiality in the provision of counseling services.
- Demonstrated experience related to the essential duties, knowledge, skills, and abilities of a Housing First Case Manager preferred.
- Must be proficient in record keeping, tracking, maintaining, and reporting statistics according to housing program guidelines and requirements.
- Valid Colorado driver’s license, clean driving record, and must carry personal vehicle insurance.
PREFERRED QUALIFICATIONS:
- Three (3) years of supervisory or management experience preferred.
- Lived experience is preferred, with an understanding of challenges related to homelessness, mental illness, and substance abuse.
- CAS (Colorado Addiction Counselor) or CAT (Certified Addictions Tech) certification is preferred.
- Knowledge of Mental Health Diagnosis and addictions counselling, experience or certifications (such as LAC) preferred.
- Experience working for a non-profit organization.
- Spanish speaker preferred but not required.
PHYSICAL DEMANDS:
While performing job duties, the employee will be regularly required to sit and stand for the duration of their shift; and talk or hear when interacting with various individuals and groups. The employee is occasionally required to walk throughout the facility and to/from vehicles and properties. The employee occasionally is required to use hands to finger, handle, or feel to operate the computer or telephone; and may need to lift and/or move up to 25 pounds.

While performing the duties of this job, the employee may be exposed to airborne viruses, bacteria, or other bodily pathogens carried by residents. The employee is occasionally exposed to outside weather conditions and fumes or airborne particles when driving from one HPP location to another or participating in an outdoor resident social activity. The noise level in the work environment can range from quiet while in a private office to loud while driving, attending a large residential social event, or visiting resident common areas where there are televisions and equipment operating, phones ringing, light to heavy traffic, and people talking.
COMPENSATION AND BENEFITS
The pay range for this position is $55,000 – $60,000 annually; this is a full-time, exempt position.
