Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The FNP will play a crucial role in providing comprehensive patient care across various settings, including critical care and memory care environments. This position requires a strong clinical background, excellent communication skills, and the ability to manage complex patient cases effectively. The ideal candidate will be committed to delivering high-quality healthcare services while adhering to best practices and regulations.
Responsibilities
- Conduct thorough patient assessments, including physical examinations and health histories.
- Develop and implement individualized care plans based on patient needs and evidence-based practices.
- Manage acute and chronic health conditions, providing ongoing monitoring and adjustments to treatment as necessary.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care, including case management and utilization management.
- Provide education and counseling to patients and families regarding health promotion, disease prevention, and treatment options.
- Perform procedures as needed, including Botulinum toxin treatment and coding for various medical services.
- Stay informed about current trends in healthcare, including workers' compensation law and hospital medicine practices.
- Participate in quality improvement initiatives to enhance patient outcomes within the practice.
Qualifications
- Master’s degree in Nursing from an accredited program with a focus on Family Nurse Practitioner training.
- Bilingual (_English/Spanish preferred)_
- NP license (must have experience in family medicine, including pediatrics)
- Full prescribing ability, including DEA
- 2+ years of independent practice experience, including routine wellness care, chronic care management, and urgent care visits
- Nurse Practitioner Certification as an Adult, Family or Geriatric Nurse Practitioner from either ANCC or AANP required or ability to obtain
- Current certification as a Family Nurse Practitioner (FNP) by a recognized certifying body.
- Active nursing license in the state of practice.
- Experience in critical care or working within a Level II Trauma Center is preferred.
- Knowledge of memory care principles and practices is advantageous.
- Familiarity with case management processes and utilization management strategies.
- Strong clinical skills with the ability to work independently as well as part of a team.
- Excellent communication skills, both verbal and written, with an emphasis on patient education.

Pay: $119,637.00 - $124,726.00 per year
