Overview
We are seeking an experienced and compassionate Registered Nurse (RN) to conduct Activities of Daily Living (ADL)-based assessments for Veteran and non-Veteran clients. This role includes completing both initial assessments and ongoing monthly reassessments to evaluate changes in functional status, ensure appropriate levels of care, and support client safety and independence. The ideal candidate is highly skilled in clinical observation, documentation, and working independently in home and community settings.
Key Responsibilities:
- Conduct comprehensive initial ADL assessments for Veteran and non-Veteran clients in home or community-based settings
- Perform monthly reassessments to monitor changes in condition, function, and care needs
- Evaluate functional abilities including bathing, dressing, grooming, toileting, mobility, transferring, and feeding
- Assess instrumental ADLs (IADLs) such as meal preparation, medication management, housekeeping, and transportation as appropriate
- Identify safety concerns, fall risks, and unmet care needs, updating recommendations accordingly
- Accurately document all findings and complete assessment reports within required timeframes
- Communicate significant changes or concerns to appropriate care team members
- Ensure compliance with all clinical, regulatory, and organizational standards
Qualifications:
- Active and unrestricted Registered Nurse (RN) license in South Carolina
- Minimum of 10 years of nursing experience required
- Extensive experience in home health, hospice care and homecare
- Experience working with Veteran populations strongly preferred
- Strong knowledge of ADL/IADL assessment frameworks and functional evaluation tools
- Excellent clinical judgment, observational, and documentation skills
- Ability to work independently and manage a flexible schedule
- Strong communication and interpersonal skills
