Responsibilities
- Provide compassionate assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ conditions, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) systems
- Assist with medication administration following prescribed schedules and medical terminology guidelines
- Perform vital signs assessments such as blood pressure, pulse, temperature, and respiration rates to ensure patient stability
- Support clients with specialized needs such as Alzheimer's care, dementia care, hospice care, and working with individuals with developmental disabilities or disabilities
- Help with medical procedures including catheter care, wound care, and end-of-life comfort measures while adhering to home & community care regulations and HIPAA privacy standards
- Assist with household tasks like cleaning, laundry, meal preparation, and ensuring a safe environment for long-term or live-in employment situations
- Use equipment such as Hoyer lifts or other mobility aids safely to prevent injury during transfers or heavy lifting tasks
- Maintain detailed medical documentation and records to ensure compliance with healthcare standards and legal requirements
Skills
- Strong patient observation skills to detect subtle changes in health or behavior
- Experience working in assisted living facilities, nursing homes, or home healthcare environments with geriatric patients
- Proficiency with EMR (Electronic Medical Records) systems and medical documentation practices
- Knowledge of medical terminology related to home health services and end-of-life care
- Ability to assist with ADLs, medication administration, catheter care, and vital signs monitoring accurately and confidently
- Familiarity with hospice care protocols, dementia care techniques, memory care strategies, and working with individuals with disabilities or developmental challenges
- Certification in CPR (Cardiopulmonary Resuscitation) and Basic Life Support (BLS), along with first aid training for emergency response readiness
- Understanding of home health regulations and HIPAA compliance to protect patient privacy and safety
- Physical stamina for activities such as heavy lifting, assisting with transfers using Hoyer lifts or other equipment, and providing long hours of caregiving support
- Compassionate caregiving approach combined with excellent communication skills to foster trust and rapport with clients and their families
