Position Summary :
The Occupational Therapist (OT) in Home Health provides skilled therapy services to patients in their home environment to promote independence, safety, and improved quality of life. The OT evaluates patient needs, develops individualized care plans, and delivers evidence-based interventions focused on activities of daily living (ADLs), functional mobility, and adaptive strategies. This role collaborates with physicians, nurses, rehabilitation team members, and caregivers to support patient progress, recovery, and long-term health goals. The OT also provides education on home safety, use of adaptive equipment, and caregiver training to maximize patient outcomes and prevent rehospitalization.
Job Responsibilities:
- Conduct comprehensive patient assessments in the home environment to determine functional abilities, limitations, and therapy needs.
- Develop and implement individualized treatment plans to improve performance in activities of daily living (ADLs), mobility, and overall independence.
- Provide therapeutic interventions, including exercise programs, adaptive techniques, and training in the safe use of assistive devices or adaptive equipment.
- Educate patients and caregivers on strategies to enhance safety, prevent falls, and optimize the home environment.
- Monitor patient progress and adjust care plans as needed to ensure achievement of therapeutic goals.
- Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals to coordinate patient care.
- Document all patient care, progress notes, and communication in accordance with agency policies, regulatory standards, and timeliness requirements.
- Participate in care conferences and provide input regarding patient status, progress, and discharge planning.
- Maintain compliance with professional, ethical, and regulatory standards, including infection control and patient confidentiality.
Support patients and families with education, encouragement, and resources to promote long-term health and independence.

Job Qualifications:
- Current OT license in the State of Florida
- Current CPR/BLS
- Minimum of 1 year of clinical experience; prior home health experience preferred
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Strong knowledge of occupational therapy principles, treatment methods, and adaptive equipment.
- Excellent clinical assessment, problem-solving, and critical-thinking skills.
- Ability to work independently and manage a flexible schedule in a home setting.
- Strong interpersonal and communication skills to effectively interact with patients, caregivers, and interdisciplinary team members.
- Proficient in documentation and use of electronic medical record (EMR) systems.
