Overview
We are seeking dedicated and compassionate Nurses, both Licensed Practical Nurses (LPN) and Registered Nurses (RN), to join our healthcare team. In this vital role, you will provide comprehensive patient care across various settings, including acute care, long-term care, assisted living, hospice, adult day or CORRECTIONAL CONTRACT positions or shift bundles , VA Positions . Your expertise will support patient recovery, comfort, and well-being through skilled assessments, medication administration, and collaborative care planning. The ideal candidates will demonstrate a strong commitment to quality nursing practices, adhere to HIPAA regulations, and possess a broad range of clinical skills to serve diverse patient populations.
Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications via injections, IV infusions, catheterizations, and other routes while adhering to aseptic techniques and safety protocols.
- Manage complex cases such as dementia care, Alzheimer's care, memory care, pain management, and infectious disease treatment.
- Utilize electronic health record (EHR) systems such as Epic or Cerner for accurate medical documentation and medical records management.
- Perform specialized procedures including tube feeding, venipuncture, suturing, wound care, and ventilator management.
- Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of post-acute or outpatient services.
- Provide triage assessments in urgent or emergency situations; support emergency medicine and Level I Trauma Center protocols.
- Support patients with developmental disabilities or those requiring specialized senior care in assisted living or nursing home settings.
- Ensure compliance with HIPAA standards while maintaining patient confidentiality and delivering high-quality patient care.
- Educate patients and families on health coaching topics such as nutrition, medication management, or chronic disease management.
Requirements
- Valid licensure as an LPN or RN in the applicable state; current CPR/BLS certification required.
- Prior experience in acute care settings such as ICU, PCU , Telemetry or emergency room environments.
- Demonstrated expertise in critical care procedures including ventilator management and cardiac catheterization support.
- Experience working with EMR/EHR systems.
- Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures, and medical terminology.
- Familiarity with specialty areas such as geriatrics, hospice care, infectious disease management, pediatric nursing (including neonatal ventilator use), or behavioral health is advantageous.
- Strong assessment skills with the ability to perform diagnostic evaluations and interpret laboratory results.
- Excellent communication skills to effectively collaborate within multidisciplinary teams and educate patients.
- Ability to adapt quickly in fast-paced environments while maintaining attention to detail and safety standards.
