What You'll Do
As a Physical Therapist, you will provide skilled home health services ordered by the attending physician while delivering compassionate, patient-centered care. Responsibilities include:
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
- Complete admissions and initial evaluations within 48–72 hours of referral.
- Perform OASIS assessments and update care plans as patient needs evolve.
- Deliver evidence-based interventions to improve mobility, reduce pain, restore function, and maximize independence.
- Educate patients and caregivers on diagnosis, treatment plans, safety, fall prevention, and home exercise programs.
- Train patients on the proper use of walkers, canes, wheelchairs, braces, crutches, and other assistive devices.
- Monitor patient progress, document outcomes accurately, and communicate changes to physicians and interdisciplinary team members.
- Coordinate appropriate admissions, transfers, discharges, and outpatient therapy services when necessary.
- Help reduce avoidable hospital readmissions through proactive patient education and early intervention.
- Ensure compliance with all regulatory, agency, and documentation standards.
Qualifications
- Current Physical Therapist license in the state of employment.
- Minimum of one year of acute care physical therapy experience required.
- Home health experience preferred.
- Strong clinical judgment and decision-making skills.
- Excellent communication and interpersonal abilities.
- Detail-oriented with exceptional organizational and time management skills.
- Ability to work independently while collaborating effectively with an interdisciplinary care team.
- Valid driver's license and current auto insurance with your name listed as the insured driver.

Work Location: On the road
