Hendersonville Health & Rehabilitation is looking for a motivated Minimum Data Set (MDS) Coordinator to join our compassionate team of caregivers. The MDS Coordinator completes all assessments and care plans for residents using the Resident Assessment Instrument (RAI) process in accordance with regulation, and company standards. The MDS Coordinator reviews and coordinates Payment Driven Payment Model (PDPM) and Case Mix data for the facility to ensure appropriate reimbursement.
Current license as a Registered Nurse or Licensed Practical issued by the State Board of Nursing
Minimum 2 years’ experience with the RAI process
Strong Knowledge of PDPM and State Case Mix Time weighted Reimbursement payment system
Knowledgeable of the nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care
Effective verbal and written communication skills
Ability to communicate at all levels of the organization and work well within a team environment in support of company objectives.
Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
Function independently, and have flexibility, personal integrity, and the ability to work effectively with employees and vendors.
Demonstrate basic to intermediate skills in Microsoft Word, Excel, Power Point, and Internet navigation, preferred.
High level of professionalism with the ability to maintain confidentiality.
Coordinates the completion of the MDS by all disciplines
Maintains and follows a schedule of due dates for all MDS
Reviews the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy.
Verifies that assessments have been transmitted and approved by the State in a timely manner.
Corrects any rejected records and prepares them for re-submission.
Completes Care Area Assessments (CAAs) according to state guidelines
Formulates Resident Care Plan along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame
Schedules and conducts Care Plan meetings on a regular, timely basis
Communicates the Care Plan to appropriate staff members and notifies supervisor if noncompliance by staff is noted
Works with all in-house and ancillary departments to assure understanding and compliance with the Care Plan
Performs other related duties as directed by the DON and/or Administrator
