You Might Be a Great Fit If You:

Conduct comprehensive physical therapy evaluations to assess functional needs, develop individualized treatment plans, provide evidence-based interventions, and monitor progress to maximize mobility, safety, and independence.

Deliver skilled physical therapy services in accordance with physician orders and the CMS Conditions of Participation (CoPs).

Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.

Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize mobility, functional outcomes, and fall prevention.

Educate patients and caregivers on therapeutic exercises, gait training, transfer techniques, fall prevention, assistive devices, and individualized Home Exercise Programs (HEPs) to promote safe mobility and recovery.

Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team.
Requirements
What We're Looking For:-
Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program.

Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing.

Home health experience preferred (minimum one year).

Strong clinical judgment, critical thinking, and problem-solving skills.

Excellent communication, interpersonal, and patient education skills.

Ability to work independently while collaborating effectively with an interdisciplinary team.

Reliable transportation, a valid driver's license, and current auto insurance.
