Job Summary We are seeking an energetic and detail-oriented Intake Coordinator to join our non-profit healthcare team. In this vital role, you will serve as the first point of contact for members and clients referred to our CalAIM services, managing their initial inquiries, scheduling appointments, and ensuring a smooth onboarding process. Your proactive communication and organizational skills will help manage a high volume of calls, distribute members into appropriate services, and support the efficient operation of our team. This paid position offers an exciting opportunity to contribute to whole-person, trauma-informed care and administrative excellence in a dynamic community health setting.
Duties
- Conduct a high volume of outbound and inbound calls to explain the program, confirm interest, gather preliminary information, and successfully enroll members.
- Receive, track, and process incoming faxes and referrals from managed care plans (MCPs), providers, community-based organizations, and internal staff.
- Assess and distribute enrolled members into the appropriate service tracks, including Community Health Worker (CHW), Community Supports (CS), or Enhanced Care Management (ECM) services.
- Complete comprehensive intake assessments and eligibility screenings according to CalAIM ECM criteria.
- Facilitate warm handoffs to the Program Manager and Care Team Manager for member distribution, as well as to Lead Care Managers, Housing Navigators, or Community Health Workers for ongoing service coordination.
- Attend daily morning huddles to align with the team on priorities and proactively work alongside colleagues to tackle the incoming referral queue.
- Schedule initial appointments and follow-up calls with new members, utilizing in-person or telephonic engagement.
- Maintain accurate and strictly up-to-date documentation in both Google Sheets and the Electronic Health Record (EHR) system.
- Ensure strict compliance with HIPAA, CalAIM, and organizational policies related to privacy, confidentiality, and data security.
Requirements
- High school diploma or equivalent; Associate's Degree or higher preferred.
- 2+ years of prior experience in a medical office, health or social services, case management, or care coordination preferred.
- Strong customer service skills with excellent phone communication abilities and the capacity to engage diverse populations compassionately.
- Proficiency with computers, specifically navigating Google Sheets, Microsoft Office Suite (Word, Excel, Outlook), and Electronic Health Record (EHR) platforms.
- Knowledge of health regulation policies, Medi-Cal eligibility, and CalAIM ECM/Community Supports (or a strong willingness to learn).
- Office experience demonstrating organizational skills, attention to detail, and the ability to multitask effectively.
- Must complete a CHW Certification Course, if needed.
- Preferred: Bilingual (e.g., Spanish, Hmong, Vietnamese), familiarity with trauma-informed care/motivational interviewing, and experience working with individuals experiencing homelessness or complex health needs.
