Hearts of Hope is looking to hire an experienced Service Coordinator to join our team to provide NM ID/DD Waiver services to our clients. Ideal candidate should be detail-oriented, compassionate, and person-centered focused to ensure clients receive the services they need and deserve.
A Service Coordinator’s responsibilities include working directly with clients and their team to ensure that their respective service plans are followed and are meaningful. A Service Coordinator will be responsible for creating required reports within preestablished deadlines and to conduct monthly face-to-face home visits.
Requirements and skills
- Proven work experience as an ID/DD Waiver Service Coordinator or similar role
- Strong work ethic and service skills
- Excellent communication and interpersonal skills
- Stay up to date with services, policies and regulations.
- Strong analytical skills
- Excellent organizational skills and attention to detail
