Home Therapy Innovations (HTI) is seeking a dedicated and compassionate PRN Physical Therapist Assistant to join our team. We are a dedicated and growing home health contracting company that partners with agencies to provide top-tier rehabilitative care to patients in their homes. Our mission is to ensure patients achieve their highest functional potential in the comfort of their home environment.
Responsibilities
- Provide physical therapy treatments as directed by the supervising Physical Therapist.
- Follow individualized care plans and maintain accurate documentation of patient progress and treatment outcomes in compliance with medical standards.
- Educate patients and caregivers on exercises and techniques to improve mobility and function.
- Collaborate with the healthcare team to ensure quality and continuity of care.
- Travel to patients' homes within designated service areas.
- Provide education to patients and their families regarding treatment protocols and home exercise programs.
Requirements
- Valid state licensure to practice as a Physical Therapist Assistant.
- Current CPR certification.
- Reliable transportation and valid driver’s license.
- Must carry liability insurance.
- Excellent communication skills for effective interaction with patients, families, and healthcare team members.
